1. This Invitation for Bids (IFB) follows the General Procurement Notice (GPN) for this Project that appeared in Development Business online on May 25, 2004 (Notice Number WB1240-633/04), print issue Number 633 of June 30, 2004 and as updated by issue no. 677, of April 30, 2006.
2. The Republic of Macedonia has received a Loan from the International Bank for Reconstruction and Development (IBRD) in the amount of US$ 10 million equivalent toward the cost of the Health Sector Management Project and it intends to apply part of the proceeds of this loan to payments under the Contract for Procurement of medical equipment, medical and office furniture, training models and consumables for clinical skills for 17 rural ambulances in municipalities trough Macedonia and Skopje Family Medicine Educational Center Premises.
3. The Project Coordination Unit on behalf of the Ministry of Health of the Republic of Macedonia now invites sealed bids from eligible and qualified bidders for delivery of medical equipment, medical and office furniture, training models and consumables for clinical skills for 17 rural ambulances in municipalities trough Macedonia and Skopje Family Medicine Educational Center Premises, consisting of four (4) lots:
LOT 1: Medical Equipment
ECG Machine (17 units), Stethoscope with sphygmomanometer (17 units), Otoscope (17 units), Ophthalmoscope (17 units), Combined oto-ophthalmoscopes with a set of spare lamps (15 units), Peak flow meter for adults and children (17 units), Peak flow meter for adults (20 units), Alexander syringe for ears cleaning (19 units), Glycometers (47 units), Strips for glycemia measuring (1000 units), Sterilizer (17 unit), Inhalator (17 units), Electrical inhalers (5 units), Reanimation set with tubes (17 units), Small surgery set I (17 units), Small surgery set II (10 units), Kidney trays for medical sterilization kit (34 units), Kit-intervention set (17 units), Infusion stand (17 units), Lamp-reflector for checkup with wheels (17 units), Neurological hammer (22 units), Glass with metal spatulas I (85 units), Glass with metal spatulas II (85 units), Glass bottles for spiritus (17 units), Speculums (5 units), Tuning forks (5 units), Automatic external defibrillator (AED) (1 unit), Mercury sphygmomanometers with stethoscopes (5 units), Negativoscopes (5 units), Monofilaments for sensorium examination of diabetic foot (10 units), Spacers for asthma-inhalers with mask (5 units), Spacers for asthma-inhalers without mask (5 units), Ambu bag (5 units), Airway for resuscitation (10 units).
LOT 2: Medical and Office Furniture
Folding bed (17 units), Static bed I (34 units), Static bed II (4 units), Screen (17 units), Chair with set screw (34 units), Refrigerator with thermostat (17 units), Movable medical cart (4 units), Stand for UV Sterilizer (17 units), Metal cupboards with possibility for locking, mounted on trestle (51 units), Cupboards with combination metal-glass, with possibility for locking, mounted on trestle (4 units), Office desk (34 units), Office chair (34 units), Chair for patient’s checkup (34 units), Bench for the waiting room with separate seats (51 units), Bin for medical waste (51 units), Bin for waste with lid (68 units).
LOT 3: Training Models for Clinical Skills
Artificial skin (20 units), Model for breast examination (1 unit), Model for prostate and rectum examination (2 units), Model for gynecological bimanual examination (1 unit), Model for gynaecology for speculum examination (1 unit), Model for HLR-adult (1 unit), Model for HLR-child (1 unit), Model for ear examination-otoscopy (1 unit), Model for eye examination-ophthalmoscopy (1 unit), Model of male genital organ for catheterisation (3 units), Model of hand for intravenous infusion (5 units), Model for diabetic foot (5 units), Model for heart auscultation (1 unit).
LOT 4: Consumables for Clinical Skills
Surgical sutures (5 boxes of 24 pieces each), Gauzes (200 units), Medical Alcohol (10 units), Medical Cotton (10 units), Latex gloves (20 boxes), Surgical gloves (20 pairs), Air Spatulas (500 units), Microbiology cotton swabs (20 units), Citobrush (500 units), Objective lenses -microscopic (15 units), Contact gel for ECG (10 units), Physiological solution (10 units), Sol.Salbutamol (10 units), Test stripes for urine analysis with 5 parameters analysis as a minimum (1000 units), Amp.glucose (10 units), Amp.vit C (10 units), Test tubes (10 units), Foley urinary catheter I (5 units), Foley urinary catheter II (5 units), Infusion systems (20 units), Mini systems for infusion (20 units), Syringe a 2ml (20 units), Syringe a 5ml (20 units), Syringe a 10ml (20 units).
Delivery period is maximally forty five days after signing of the contract.
4. Bidding will be conducted through the International Competitive Bidding (ICB) procedures specified in the World Bank’s Guidelines: Procurement under IBRD Loans and IDA Credits January 1995, revised January and August 1996, September 1997 and January 1999 and is open to all bidders from Eligible Source Countries as defined in the Guidelines that meet the following minimum qualification criteria:
For LOT 1:
· Bidders must be able to prove financial stability and solvency by submitting their audited annual financial statements for the last three years showing average annual turnover no less than US $ 500,000.00 equivalent over the period of the last three years.
· Bidders (Manufacturer and the Agent if the latter submits the bid under manufacturer’s authorization as per form 6) must demonstrate that it has experience in delivery of similar equipment in kind and quantity during past three years.
· Bidders must provide documentary proof that they have been in their current form of business for at least three years.
· Bidders should have at least three (3) years of similar experience in manufacturing, supplying, installing and supporting, the required equipment.
· Bidders must provide documentary proof of the existence, or must commit themselves (to submit a Statement) to enter into an agreement with an established facility upon award of the Contract to at least one service facility within the territory of The Republic of Macedonia. This facility which shall operate directly under the authority and the responsibility of the Supplier, will be able to perform full servicing and repair of the equipment and will have at their disposal all the spare parts and consumables needed, or will be able to procure them as soon as possible in a reasonable time but no longer than three (3) months upon the written notification by the Purchaser to the Supplier. The service facility must be fully operational at the latest by the time the goods are being delivered.
For LOT 2:
· Bidders must be able to prove financial stability and solvency by submitting their audited annual financial statements for the last three years showing average annual turnover no less than US $ 200,000.00 equivalent over the period of the last three years.
· Bidders (Manufacturer and the Agent if the latter submits the bid under manufacturer’s authorization as per form 6) must demonstrate that it has experience in delivery of similar equipment in kind and quantity during past three years.
· Bidders must provide documentary proof that they have been in their current form of business for at least three years.
· Bidders should have at least three (3) years of similar experience in manufacturing, supplying, installing and supporting, the required equipment.
For LOT 3:
· Bidders must be able to prove financial stability and solvency by submitting their audited annual financial statements for the last three years showing average annual turnover no less than US $ 300,000.00 equivalent over the period of the last three years.
· Bidders (Manufacturer and the Agent if the latter submits the bid under manufacturer’s authorization as per form 6) must demonstrate that it has experience in delivery of similar models in kind and quantity during past three years.
· Bidders must provide documentary proof that they have been in their current form of business for at least three years.
· Bidders should have at least three (3) years of similar experience in manufacturing and supplying, the required models.
For LOT 4:
· Bidders must be able to prove financial stability and solvency by submitting their audited annual financial statements for the last three years showing average annual turnover no less than US $ 10,000.00 equivalent over the period of the last three years.
· Bidders (Manufacturer and the Agent if the latter submits the bid under manufacturer’s authorization as per form 6) must demonstrate that it has experience in delivery of similar consumables in kind and quantity during past three years.
· Bidders must provide documentary proof that they have been in their current form of business for at least three years.
· Bidders should have at least three (3) years of similar experience in manufacturing and supplying, the required consumables.
In addition to previous, for all Lots:
· In the case of the joint venture, qualifications of joint venture partners are added together.
· Qualifications of subcontractors are not added in any circumstances.
· If an Agent submits bids on behalf of more than one manufacturer, unless each bid is accompanied by a separate Bid Form for each bid, and a bid security required for each bid and authorization from respective manufacturer, all such bids will be rejected as non-responsive.
5. Interested eligible bidders may obtain further information from Project Coordination Unit of the Ministry of Health and inspect the Bidding Documents at the address given below from 0900 to 1500 hours during working days.
6. A complete set of Bidding Documents in English may be purchased by interested bidders on the submission of a written Application to the address below and upon payment of a nonrefundable net fee US $ 150 or equivalent in Macedonian Denars (MKD), applying the median exchange rate of the National Bank of the Republic of Macedonia on the day of payment prevailing.

7. Bids must be delivered to the address 1 at or before April 28, 2010, 12:00 hrs NOON Skopje Time. Electronic bidding will not be permitted. Late bids will be rejected.
